I am sorry to hear about this. If she doesn’t want to go to the doctor, there is not much that you can do to help her.
Call any doctor or clinic and talk to them and explain what is going on. Best answer would be to go to the doctor. In the meantime, she needs to eat and will need to go to the hospital to get an IV if she doesn’t! Remind her of this and hopefully it will scare her enough to help herself until she gets to the doc!!
JEEZ CALL A DOCTOR… I’M 14 AND I KNOW THAT o.0
This has nothing to do with her diabetes. Call a doctor.
Do some research on diabetes and find out what other people with diabetes recommend.
Good place to start is a diabetes resource site:

It sounds like your great aunt may not have been taking care of herself as well as she should. Get her some medical care.
She may have lost a lot of muscle with the weight loss which may explain the weakness and the lack of appetite. She needs to recover and gain strength. She seems to need your help to renew her health.
Best Wishes.
